i tried answering it but everytime i hit "done" it says "this question requires an answer" for #5. even though i answered it. hell i checked every box to see if itll go through with no luck.
i had the same problem but when i checked only one box it let me submit

Yeah I chose all the answers for #5 because I think it is a combination of all of them. I think a rewording of the question would help. What is the #1 reason...etc.
But yeah I filled it out.
