You have to sand the rust spots to the bare metal surface removing it ALL. If it the rust has eaten completely through the metal, you will have to rig a patch (probably would involve some slight welding if you have to patch it) then you would grind the welding spots off, bondo if there are low spots (no deeper than 1/8 of an inch), sand everything smooth, then prime and seal it and then touch up paint.
If you just touch up over the rust it wont do anything, only hide the damage for a little while, and risk worsening the rust there is and possibly spread it.

Yea Celicaboy is right. You need to sand the paint down to bare metal, I would also treat the metal with a product called OSFO or any rust eating chemical. Then prime it with a 2 part primer, spray a sealer, and repaint the door.
Chances are the rust was caused by a factory mistake in the paint job, or the door has had bodywork done in that area before and someone skipped a step. Hope it works out for ya.
That looks like its going to be pretty well developed rust spread all along the wing, unless its an error on my monitor and it isnt as bad as it looks, I`d go for a new front wing. On the back, grind out as much as you can, all of it if possible or like already mentioned it will come back and just spread
